I, ________________________________ hereby authorize KINDERMOMS it's legal representatives and assigns, licensees, those for whom KINDERMOMS is acting, and those with it's authority and permission, the voluntary, irrevocable and absolute right and permission to copyright and use, re-use and publish, and re-publish photographic images and videos in all forms, of my child in which he/she may be included, in whole or in part, without restriction as to changes or alterations, in color or otherwise made through any media type for art, advertising, trade or website, or any other purpose whatsoever.  I further waive any rights of compensation or ownership thereto of such images as described herein. I understand that KINDERMOMS cannot control the unauthorized use by persons other than KINDERMOMS of my child's name or image once such name or image is published.  Any claim I may have concerning unauthorized publication of my child's name or image must be pursued by me against the unauthorized user.  KINDERMOMS disclaims any responsibility for such unauthorized use. I consent to the use of any printed matter in conjunction therewith. I hereby waive any right that I may have to inspect or approve the finished product or products or the advertising copy or printed matter that may be used in connection therewith or the use to which it may be applied.
